Memory Assessment and Treatment Service in Trafford (MATS)
Trafford MATS provides a service for Trafford residents aged 18 years and over who require assessment, diagnosis and treatment of memory problems. Service users may also receive an occupational therapy or psychological assessment or be referred on to other services.
The service operates from 9am to 5pm, Monday to Friday excluding bank holidays.
How to Make a Referral
Referrals on the GP electronic referral form or in writing should be faxed or emailed to MATS.
Referrers are expected to provide the results of dementia screen investigations.
i.e. the following blood results:
- Full blood Count (FBC)
- Biochemistry tests
- Urea and electrolytes
- Bone profile
- Glucose HB1C
- Liver Function & renal tests
- Thyroid function test
- Serum vitamin B12 and Folate levels
- eGFR
Referrers are also asked to provide a rationale for referral, a summary of past medical and psychiatric history and a list of current medication.
If an ECG has been undertaken within the previous 6 months this should be included.
Timescale of Response
The service aims to assess people within 6 weeks of acceptance of referral and deliver diagnose within 12 weeks.
A letter will be sent to the GP by the nurse or occupational therapist following assessment should the service user not require or want a further appointment with a psychiatrist.
If an appointment with a psychiatrist is arranged the letter detailing the outcome of the assessment will come from the psychiatrist.
Pre- and Post-diagnostic Support
Pre- and post-diagnostic support in Trafford is offered via AGE UK. All service users are referred routinely but may opt-out if desired.
